You shouldn't need to use SoftTH for your single monitor. Try the latest Flawless Widescreen plugin, that's probably where your issue is coming from. (Note: I don't own the game, but that's what Flawless Widescreen is for, making sure your FOV and aspect ratio is correct.)

Do you need to use 4K res? If not, you can make a EDID Mask to make your computer think that the 27"rs are 2560x1440 max and get your desired resolution.

Kinda of confused on your setup.
But as long as you have all 4 LCD's attached to your AMD GPU (5000 series or higher), you should be able to make an eyefinity group.
If you have a DP connection, you may want to look into getting a DP 1.2 MST HUB to connect everything up to.

Do this, turn on monitoring (I use Afterburner) and see if your video card is using its 3D or 2D Clocks.
It may be a bug. You may also want to try to update your drivers.
Uninstall them normally, then use DDU (Display Driver Uninstaller), and then install the latest drivers. See if the lag still ...

We got a huge RX 480 Update here. Looks like the 4GB cards are in the wild now. All of these are Eyefinity 5 Gaming Cards. Looks like a nice mix between wanting 2DP/2HDMI or 3DP/1HDMI

I like the trend to 3x DP on a GPU
I feel you on that. But I do wish that they would just put 6xminiDP on a high end card... Although that requires a high end card. Bring it AMD.

Why are there no custom RX 480's? I was thinking of upgrading my or my wife's rig to one. (She is still suing a my old 5870).
