is there a Windows tool to position monitors exactly?

Not exactly a gaming question. I hope it's okay to post this here.

This dialog only has a precision of 35 pixels using arrow keys or mouse:

It would be nice to adjust these positions to the pixel, so that windows and pointer movements line up across monitors. Is there a way to position these using the registry or some freeware tool?

Uhhhh.... good question.
In the Former AMD CCC there was a "Preset" which saved Monitor Position. You could manually edit that file via text editor and change Monitor position.
This feature is no longer existend in new AMD Crimson drivers.

What kind of screenshot is this even? Some Skin for Windows?

This is the Windows 7 "Screen Resolution" dialog with Aero disabled.
