Civ V multimonitor UI

Playing Civ V at 5760x1080 is horrible. The interface forces your head to move from one extreme to the other. I have looked all over the internet for a mod that fixes this issue (including on here) and not found anything. Ideally I would like the 21:9 (1920x1080) user interface over the 48:9 (5760x1080) game-play. But I would settle for anything which wouldn't give me a bad neck tbh.

If the mod didn't affect achievements through steam or online play it would be even better but that is secondary.
