This is probably only a problem for a small minority, but I play on a 40" Samsung TV and here's what happens (tested with my old disc version and the Steam version):
The menu always starts up in 800x600 resolution. Once the game starts it will change to the chosen resolution.
The problem is that due to the TV (LA40C650) not properly supporting 800x600, it changes its mode to 1920x1080x24hz. Unfortunately once the game starts it stays in 24hz leading to overscan and low perceived framerate.
If there was any way to force the menu to display in 1080p or similar, I think that would fix the issue. If anyone knows how, or can implement this (jackfuste?) I would be very grateful.

Don't know why this didn't occur to me before, but changing to windowed mode in Documents\Tilted Mill\Children of the Nile\cotn.ini will fix all the above issues.
But of course fullscreen would be a lot better if you're still willing to take a look.

I locked it at 60Hz - available from the PC Gaming Wiki Community Files (search there)

That worked beautifully (after I deleted the Tilted Mill folder from Documents). Interestingly after quitting back to the main menu it remains in 1080p now rather than dropping back down to 800x600.
