three 30" monitors displayport windows login problem

I recently upgraded to a new computer and was trying to use a single ATI card to run all three of the 30" monitors. The ATI has two DVI connections that I can directly connect to the monitors and one mini DIsplayport that needs an adapter. Since I already had the apple adapter, I have been trying to use it with my third monitor. The computer works fine with two monitors but has a peculiar problem when I try to us the third. When I go to the windows user login screen, the third monitor on the adapter looses signal and all I get on the other two is the arrow cursor. Some describe this as "black screen of death" (KSOD). If I power cycle the adapter by unpluging/reinserting the usb power cord the third monitor wakes and I am able to log in.

Any ideas? If I cannot figure out a simple solution I will try a the hp displayport to dvi-d active adapter or an aftermarket adapter (accell, monoprice, bizlink, ...).

It's the apple adapter, I have 3 running each of my 30"rs. I just power cycle the adapter anytime I have issues. This is made easy by powering the adapters using the USB hubs on the monitors. Turn off monitor->disconnect. Turn on monitor->reconnect.
