Hi everyone, I registered to thank you all for the great fixes (especially jackfuste), and to answer this question specifically.
I had the same problem with the V2 version of the FPS unlocker, couldn't get it to work in full screen. The simple fix, at least for me, was to go into the .exe properties and toggle on "disable fullscreen optimizations" from the compatibility tab.
I hope this works for you as well.
I downloaded the 4.7 exe, patched it with the 2.7 FPS fix and turned on the "disable fullscreen optimizations". However, for some reason, this still won't make it work in fullscreen (max 60 fps and hz). Any ideas?
EDIT: Forgot to mention - it does work in borderless fullscreen though, however the frame pacing seems to be all over the place...

Did some more testing and managed to get the latest fps patch to work in fullscreen and take advantage of freesync.
After applying the fps patch edit d3dx.ini, look for refresh rate, change to whatever hz your monitor supports and remove the semicolon.
Frametime is a lot more stable, everything is smooth as hell like in the original 60hz mode but with more fps now. Still testing the random block/deflection miss bug, could be my lack of skill haha.
Also love how blazing fast the game loads after fps patch.

i noticed that too . i used ssd, it seems w/ uncapped fps make loading even faster haha . hopefully help w/ harddisk too

How'd you do this? I do not have a d3dx.ini file in the folder.

I unpacked my .exe and ran the 144hz patch. It worked only in windowed mode which runs in a small window so I downloaded the "v4" .exe and it is working in fullscreen windowed mode.
I have a couple of questions. I'm pretty new to modding games. If the developers update the game on Steam will I have to reapply the mod? Also, is messing with this .exe going to get me banned or anything on Steam?

Download sekiro v4, disable texture v3 and sekiro fps patch. Unpack all files in the folder, patch sekiro.exe and then modify d3dx.ini file with the corresponding refresh rate. Don't forget to delete the semicolon - ";".
Run the game and change to fullscreen, it should work fine.
I run 3440x1440@100 hex edit +75% FOV Sekiro V4, Sekiro_FPS V 2 and Disable_textures V3. I did change d3dx.ini ;refresh_rate=60 to refresh_rate=100 Don't forget to remove the semicolon. In fullscreen I get 85-86 FPS. Windowed 77-78. A 9-10% performance improvement for fullscreen. Edit. The new nvida drivers have improvements for Sekiro. Have anyone done benchmarks?

I just surpassed that same zone and did not encountered the bug you describe - playing with patched .exe (the second to last one by Jack for 21:9), hex edit for FOV, texture removal (the one with bigger posture lines) and FPS unlocked via patch
