I have played ~15 hours of Mass Effect 3 on my current setup, using a 2560x1080 ultra widescreen display and have been using Flawless Widescreen, but it rescently stopped working. I wanted to see If I could use Nvidia DSR to improve the image, but the mouse position was incorrect once I set it up, so I turned it off and went back to my monitor's native resolution. After I did so, Flawless Widescreen stopped working on the game. It claims to be functioning, but the menus are not positioned correctly. I have attempted restarting, removing mods, undoing Coalesced edits, undoing Nvidia Inspector Modifications, and I also attempted using Widescreen Fixer, which didn't work. Anyone have an idea what is wrong? Thanks.

To start with, wondering how you have things set up:
In FWS, do you have "Enable Detection Override" checkmarked & details filled out? It's under FWS menu, Settings, Configuration, Display Detection.

If you are not using the above FWS override, that is the first thing you should do. Here are the FWS settings most likely to work.

Width: 2560
Height: 1080
Hud Width: 1920 (16:9 fake center)
Hud Height: 1080 (16:9 fake center)
Horizontal Display Count: 3 (with fake sides)
Vertical Display Count: 1
Hud X: 320 (which is 2560 minus 1920, divided by 2 = width of fake side)
Hud Y: 0

(These entries are for your 2560x1080. But we are pretending you have PLP or it likely won't work (ie fake 16:9 center, fake skinny sides). This likely gives full spread with proper 16:9 HUD size etc.)

imusrt, those are the settings I have but it isnt functioning. The HUD isnt centered and menus dont work properly.

Forgottencrow has normal no-problem 21:9 (you are welcome btw ^^).

benger, this implies your system's video driver has become corrupted. This is the likely fault, not FWS. DSR can screw stuff up internally, it's a bit rough (can act bit buggy).

Recommend next:
Turn off DSR. Uninstall your graphics card driver & install fresh. Then try game again.

Let me know result. If above fails, would recommend these next:
1. You could search web regarding video-resolution-related corruption, possibly relating to DSR. A system fix may be documented. Solution will involve somehow refreshing the corrupted area.
2. Failing that, recommend manually enforcing the resolution, to get around whatever's messed on your system. If it turns out you need this, let me know & I will explain.

imusrt is there a way to remove black bars in cutscenes :)?

No I don't think there is (FWS plugin would have to be changed, by its developer).
Interestingly, PLP rendered cutscenes in this game are pillarboxed to center (black sides). So guess 21:9 has "margin" where fake sides are located.

reinstalling the driver didn't fix it, can you tell me how to manually force the resolution?

Sorry to here that. Pretty sure corruption is cause. Wonder if other games act same? Or if game itself is caught on something.

Just in case game config corruption is the cause I would delete %USERPROFILE%\Documents\BioWare\Mass Effect 3\ BIOGame\Config\GamerSettings.ini.
This will likely create a fresh ini file with default resolution.

Failing that, force Mass Effect 3 resolution using manual method (which is SoftTH fullscreen):

EXCEPT, this change:
Under Notes, #4.B & C, regarding LUA edit: this whole step is not needed I think, if you use FWS detection override instead (like PLP, described in a previous post).
