Doesn't work for me.
I'm running 5760x1080 with eyefinity and when I enable the fix, I just get three black screens. With the fix disabled I have the game on the middle screen again.

I just made a funny discovery:
I already had tried to get the Steam Version (Witcher 2: Enhanced Edition) 5760x1080 working with Eyefinity and was almost giving up, until I realized that I had to set it to 1x19 and 5760 as h-resolution.
Then I opened the game and it worked! But the FOV was strange ...
