I was having troubles too to get this game playing in 21:9. Tried the hex edit and endep up having an error launching the game. I gave Flawless Widescreen another chance and it now works properly.

It also looks like the FOV doesn't scale correctly in the Pip-Boy picture, looks a bit zoomed in.

I hope Day 1 patch fixes some of these issues.
