I can see that you want to run 3 monitors in surround. This being the case, can I ask why you need active adapters at all?
From what I can tell, you can just run 2 dvi cables direct to 2 monitors, then use any basic cheap hdmi to dvi adapter to turn your hdmi port on ...

Update heads up: Mass Effect 3 has been patched again. The current FWS patch is missing one of the menu fixes that would break the game in certain situations - I can give more detail if needed. I will email you the same, as you mentioned in your previous post.

Minor bugs to point out for reference:
The "escape" button screen (mainly in multiplayer) is massively zoomed - this was fixed before so maybe the location moved.
When you die in game, the fov massively increases - super fish eye mode! Not game ...
