I have updated the report with details about using the new
dgVoodoo2 DirectX resolution override to play in high resolutions.
Multi-monitor doesn't seem to be possible, at least for AMD setups. AMD doesn't support adding the necessary custom resolutions to Eyefinity and I didn't have any success forcing a windowed mode with D3DWindower or DxWnd (the game's native windowed mode switched back to fullscreen when I tried to use a borderless program with it). The game can successfully produce a 5760x1080 output in single-screen fullscreen mode, so hopefully a future version of dgVoodoo2 will introduce some other feature that can be used in combination to get around this limitation.
