| Yes! It is possible with AMD. It rolled out with the Catalyst 14.6 Beta drivers. The monitors have to be in the same orientation (all portrait or all landscape), but other than that, you can mix and match a bunch of different aspect ratios. | i had the same issues like everyone else ui a bit off etc. Weird enough sometime is was working. I got it work now by deleting my dsfix.ini creating a new one and readjust my settings. | Something is wrong with the last version of Widescreen Fixer application, not the game's DLL, it is not applying the HUD fix at all, I just downgraded to V3.4.545 and it all works fine with the same Dark.Souls.dll |

Seriously thank you guys, i just played 8 hours with the fix and even though the HUD is a bit annoying, I can still play it without any problems!

I really hope you find a way to fix this game, it would look amazing in Triple screens with the art style.
I also get black screens with any triple head resolution :( |
